Guest information form not showing up correctly

I am using this free plugin that is really great and what i needed.
But some how i do something wrong and the guest form to fill in does not look correct.
The text for each data like name, email phone is all appearing in one line and not on top of each block.
U tried other templates...but same problem...
help please...what i do wrong?
