Example of embedded form

Hello, we are interested by the module for Joomla
I see that we can embed the form in other websites.
Before we buy the paid version, I would like to see how the embedded form will look like.
I already searched in this forum and on the internet, but i couldn't see any example.
So, ca someone please show me a screenshot of how an embedded form looks like ?
