Offline Payment Method - Form appearance

Is there any other version of this offline input form available, that would look at least decently aligned?

Please make sure that your template's Bootstrap version matches the Bootstrap version as configured in Solidres Config > UI Framework.

When i go to Solidres Configuration > UI Framework menu i have this setting.

In the Extensions / Templates / Styles / MyDefaultTemplate I cannot find this function anywhere?

Also Another question I have.

When I try to test OFFLINE payment with real credit card I get error?
An 0 error occured during your request.

The booking is SAVED in the Reservations in the ADMIN but in the Field: Transaction Id it says Empty



I guess here should be credit card details for administrator? or?

Where seems to be the problem?
