How to change the requirements on the Guest Information Form

Hi -

Is there somewhere to change the fields on the reservation form? There are fields we don't need, like "landline" and "VAT number" also would like to add a field if possible.

Let me know if I can edit the form where it's located, or if there is somewhere in the CMS. I also need to work on the styling for the form - it's crowded with boxes and titles on separate lines making it confusing. I can send a screenshot if requested.
