Payment Gateway Fee

Do we have to pay that extra $49 or $39 fee to set up a gateway? That is an extra fee on top of the subscription fee we paid already? Is that gateway fee optional only if we need support or for updates? Can I update on my own with out paying this fee? Can you please clarify. Thanks
